- Care intake details you provide voluntarily (e.g. patient needs, preferred contact time, insurance type). Do not include sensitive clinical detail that is not necessary to schedule a consultation.
- Usage data such as pages viewed, device, and approximate location derived from your IP address.

HIPAA notice
When MTCI provides care, certain information becomes Protected Health Information (PHI) under HIPAA. PHI is handled under our Notice of Privacy Practices, which is provided at the time of service. Submitting a form on this website does not by itself create a treatment relationship; please avoid sending detailed PHI through web forms.
Sharing
